Observations

A quick report when something does not look right and should not wait for a full checklist.

Observations list with open, in-progress, and resolved field reports

What it is

An observation is a short, free-form report: recognising and recording a specific condition, behaviour, event, or hazard. It is not tied to a template. The point is to capture it quickly, give it a category and a priority, and track it until it is resolved. Unlike a formal inspection, which is a scheduled checklist, an observation can happen at any time and focuses on the reality of daily work practices.

Categories

Categories are set up by the organisation, for example near miss incident, hazard, property damage, or unsafe behaviour. A category can:

  • Ask extra questions
  • Point at a follow-up checklist
  • Decide who is told when a report is created or marked critical

A critical report can also raise an unassigned action automatically, and some categories can refuse to be closed until a task exists.

From an observation you can

  • Move it from open to in progress, resolved, or cancelled
  • Attach photos, including on the phone while offline
  • Record a location
  • Link it to a site, an asset, or a contractor company
  • Assign an owner
  • Raise a task, start an inspection, or start an investigation (the investigation itself continues on the computer)
  • Leave notes on an activity feed

Where

Phone first, also on the computer. The computer list can be cards, a table, or a kanban board.
